Urinary tract infection (UTI), the most common bacterial infection, among renal transplant recipients (RTRs) remains a challenge particularly given increased incidence of MDR organisms including Carbapenem resistant Enterobacteriaceae (CRE). Compared to other infections, CRE infections were associated with inferior patient and graft outcomes. Paradoxically, there have been no guidelines on management in RTRs besides multiple challenges bacteriostatic nature, poor urinary concentration dose limiting adverse effects various antibiotics.
